About the job Are you a nurse with experience in palliative care who wants to try something new? Now you have the chance to support our hospice case managers in meaningful and important work where you can make a difference for those we serve. The hospice case management office is located at F O Petersonsgata in southwestern Gothenburg and consists of two experienced full-time case managers. We are now seeking a colleague who wants to work hourly and can strengthen our case management team during planned and emergency absences. A perfect assignment for you if you are perhaps studying further, are retired, or for other reasons are not working clinically right now and want to develop in a new role. With us you will process applications and assess whether patients are eligible for hospice care based on the health and care process. You decide and place patients at three hospices, one of which is run by the City of Gothenburg and two are external. The work involves collaboration with multiple parties and you gather information to get a comprehensive picture to support your decisions. Home visits to patients may also be part of the investigative work. You then follow up your work and the occupancy of the hospice beds. To help you, you have clear assessment criteria and experienced colleagues on the team to consult as needed.
