Region Västerbotten works to ensure that good health and sustainable development strengthen each other. We take responsibility for equitable welfare and for ensuring that research and innovation deliver results. The Geriatric Center is a specialist clinic for patients in need of both acute and subacute geriatric care and rehabilitation. A comprehensive assessment of the elderly patient's medical, physical, mental, and social functional capacity in illness, as well as activity capacity, is performed by a multidisciplinary team, known as Comprehensive Geriatric Assessment (CGA). Diagnostics, treatment, and rehabilitation are conducted with the patient's individual needs in focus. The target group consists of people 65 years and older with multiple medical conditions, combined with age-related consequences, who require specialized care and rehabilitation. At the Geriatric Center, various professionals work together in effective collaboration across and between departments. We focus on subacute general geriatric care and rehabilitation, as well as fast-track pathways with patient transfers from MAVA and the infectious disease clinic. We provide psychogeriatric specialized care, treatment, and rehabilitation for individuals with cognitive diseases and behavioral and psychological symptoms of dementia (BPSD) regardless of age. We also provide orthopedic geriatric acute and subacute care and rehabilitation. Additionally, we offer outpatient services including psychogeriatric clinics, osteoporosis clinics with bone density measurements, and specialist teams working in homes with individuals with dementia, multiple medical conditions, and orthopedic conditions.
