We are looking for a social worker to join our planning team at the authority unit in the Home Care Administration. Your Responsibilities As a social worker in the planning team, you will work primarily with investigating needs and making decisions for elderly residents upon discharge from inpatient care, in accordance with the Social Services Act. You will collaborate closely with other professionals in our planning team, with family members, with social workers at units in ordinary housing, and with home care and short-term care providers. In collaboration with inpatient care, you have a mandate to ensure that each resident's discharge process is safe, legally sound, and efficient. As a social worker, you are expected to be both engaged and participatory in ongoing efforts to improve work methods. Your Qualifications We are looking for someone with a degree in social work or equivalent higher education including 30 credits in social law. We ask that you attach your degree certificate from your social work education with your application. Previous experience in authority work is a merit. You should also be updated on current regulations and values within the field. Authority work involves extensive documentation, which requires good computer skills and the ability to use and benefit from various digital tools. It is a merit if you have worked in Lifecare and if you have knowledge of the National Board of Health and Welfare's investigation model IBIC. You also need good ability to express yourself in Swedish, both orally and in writing. As a person, you show interest and understanding for the residents you meet and work for good communication and collaboration with colleagues, family members, and other professionals. In the role of social worker, you need to work independently with a good ability to organize and prioritize your work efficiently. Furthermore, you are flexible and decisive, you adapt to changed circumstances, and you drive processes in a goal-oriented manner. Great weight will be placed on personal suitability. What We Offer In our planning team, you are one of four social workers who, together with the team's nurses, occupational therapists, and physiotherapists, are responsible for the discharge process. The planning team is part of the authority, which also consists of 25 social workers tasked with working with residents in ordinary housing, housing coordinators, and fee administrators.
