We are looking for a social worker to join our team who, with professional knowledge, compassion, and a sense of responsibility, is able to plan and manage long-term social care and social rehabilitation processes for adults, coordinate their work, and support clients in their daily lives. Social Care Center "Trapene" is seeking a social worker for our team who is ready to get involved in clients' daily lives, work in close cooperation with social work specialists and the care team. Your main responsibilities: perform social work with adults receiving long-term social assistance – seniors, persons with disabilities, functional and mental health disorders; identify and engage necessary resources and support in social case resolution, respecting the specifics of social work; develop and organize social care and social rehabilitation plans in accordance with the individual needs of each person; organize and coordinate cooperation with involved specialists and caregivers; create and maintain client records in electronic and/or paper form, ensuring document registration, archiving, and data protection in accordance with regulatory requirements. We will value in you: first-level higher education in social work or charitable social work; experience in the field of social work; ability to plan, manage, and organize work, ability to express your opinion clearly and with sound arguments; ability to work in a team and a sense of responsibility; good interpersonal and communication skills; knowledge of Latvian language.
