Make a difference. Every day. We are now seeking a Registered Nurse who wants to divide their time between Hospital-at-Home Lund and Internal Medicine Day Care Clinic Lund. We are part of the Acute Care and Internal Medicine operational area. At our internal medicine day care clinic, we work with follow-up care for patients who have been treated on our medical wards to prevent readmission, and we provide rapid subacute assessments for patients within specialized internal medicine. We have many heart failure patients who, following discharge from inpatient care, are followed up with us by a heart failure nurse and physician until they are sufficiently stable to be referred back to primary care. Patients may come daily for a period or for individual visits. Hospital-at-Home means that patients who need acute specialized inpatient care are offered this in their own homes instead of on a hospital ward. Hospital-at-Home is operated by Internal Medicine but in collaboration with Infectious Diseases, so both specialist competencies are present in the team. 2 position(s). DUTIES As a nurse in the day care clinic, you will work partly independently, primarily with heart failure patients in outpatient care, as well as with various medical treatments performed in connection with the specialist physician. You will also work in a team with physicians for our subacute patients, where various medical day care treatments occur. In Hospital-at-Home, the position primarily involves independent work with patients in their homes where we perform nursing care, assessments, medical treatments, and evaluations. We can manage much of the medical care that is typically performed on a hospital ward. Our patients come directly from the emergency department, from hospital wards, or directly from primary care. This may also include treatments at the hospital for our patients enrolled in Hospital-at-Home. Some digital work is included, such as video rounds with patients and coordination of patients at the hospital. The position includes day, evening, and weekend work. QUALIFICATIONS We are seeking someone who is a trained Registered Nurse with authorization to practice within Swedish healthcare. Swedish language skills equivalent to at least level C1 according to the Council of Europe scale are required. The position requires previous experience working as a nurse and a valid driver's license (category B). Specialist training in infectious diseases, internal medicine, or similar is advantageous. Experience in internal medicine, infectious disease care, or other inpatient care is also advantageous. Experience with heart failure care or relevant training in this area is also a plus. As a person, you are positive, engaged, and have a desire to develop in your profession. We also value your humble approach in your interactions with both patients and colleagues.
