About the Job Would you like to work with close patient care in patients' home environments? We are now seeking a patient-responsible nurse who wants to become part of our team at Bror Nilssons gata! Our office is located at Bror Nilssons gata 5 on Lindholmen with good transportation options—you can get to work by boat, bus, or bicycle. We currently operate on a standard schedule where you mainly work daytime hours with some evening shifts and weekend duty every fourth week. As a patient-responsible nurse, you are responsible for setting goals, planning, leading, and following up on care in consultation with your patients. Interventions are carried out as home visits, through guidance or delegation to care personnel. You will collaborate in multidisciplinary teams and with other stakeholders, both internally and externally. In your role, you will work closely with colleagues in rehabilitation, home care, and health centers. Your assignment also includes identifying and preventing risks in care as well as assessing patients' need for health and medical services. To gain the competence needed to work within municipal health and medical care, Gothenburg City offers an introduction program for new employees. The program is designed in dialogue between you and your unit manager. You will also have a mentor who introduces you to the work and provides support. About You You are a licensed nurse or district nurse; if you have other specialist education, that is a merit. It is meriting if you have previous experience in nursing, but we also welcome newly graduated nurses. The position requires that you have a B driver's license, as you drive between patient visits. The work requires that you can independently oversee, prioritize, and perform your tasks. You have the ability to work together with others and create good relationships.
