You will work full-time with our home health care mission during daytime hours on weekdays and weekends off. About Us Bromma Primary Care Clinic is a personalized primary care clinic in the heart of Brommaplan with operations within Region Stockholm. We are a smaller organization with short decision-making processes, high expertise, and close collaboration between physicians, registered nurses, and nursing assistants. We have a growing home health care mission, chronic disease clinics, and active development work where employee ideas are valued. Our home health care team currently consists of three nursing assistants, one registered nurse, and a home health care physician. One of our appreciated registered nurses is moving on to new challenges in the fall, which means we are now looking for their successor. We are therefore seeking you to take an important role in our home health care team and continue to develop our services together with us. About the Position As a registered nurse in home health care, you have a key role in daily operations. You work both independently and as part of the team to ensure safe, secure, and person-centered care in patients' homes. You serve as clinical support for our home health care nursing assistants and collaborate closely with our district physicians on planning, assessments, and follow-ups. The work is varied and combines patient visits with coordination, planning, and medical assessments. Examples of Work Tasks Plan and coordinate daily home health care. Supervise and provide clinical support to home health care nursing assistants. Conduct home visits and nursing interventions. Participate in care planning and SIP meetings. Collaborate with physicians, family members, social services, and other care providers. Manage incoming cases via LifeCare. Respond to our direct telephone line for home health care patients during part of the workday. Contribute to the development of procedures and quality within home health care. Who Are You?
