Bräcke diakoni is seeking a registered nurse for our home care team in Linköping. This is an 80% position.
You will work daytime hours, weekdays, in a role where you meet patients in their home environment and contribute to close and cohesive care. We are part of Bräcke diakoni, a mission-driven organization working toward a more humane society.
Your Role
With us, you contribute to ensuring that care functions safely for our patients in their home environment. You meet them in their ordinary residences, in Bräcke's supported housing facilities, and within our home care areas in City Syd. Many are elderly and people with disabilities. An important part of the role is therefore to monitor health over time, identify changes early, and contribute to interventions that strengthen independence and quality of life. You will be based at Ramstorp supported housing facility, where you share an office with your nursing colleagues. In your work, you guide care staff, collaborate with doctors and rehabilitation staff, and contribute nursing perspective to the development of our working methods. You become part of a multidisciplinary team where knowledge in nursing care, rehabilitation, palliative care, and cognitive disorders is utilized around the patient.
Who We Are Looking For
We seek you as a registered nurse who thrives in a role where you get to collaborate broadly and contribute your expertise. You have the ability to create an overview and plan care interventions that are health-promoting and adapted to the patient's situation. You are experienced in guiding care staff and contributing to shared prioritizations when needs change. Your interaction is clear and attentive, and you contribute to a collaborative working approach around the patient.
Your Qualifications
Registered nurse
Experience with HSL duties
Valid driver's license (category B)
You speak and write Swedish fluently and have good computer skills
Meritorious
Experience in home care services
Experience guiding staff in nursing care work
Experience in geriatrics or palliative care
Experience with the Treserva and Cosmic documentation systems
Bräcke Home Care Services – Linköping City Syd
In Linköping City Syd, Bräcke diakoni has overall responsibility for care, home care services, and rehabilitation in the home. Approximately 150 committed colleagues work together across our operations here.
In the home care team, nurses, physiotherapists, and occupational therapists work closely together, with the patient's daily life and life situation as our starting point. Around the patient, we also collaborate with Bräcke's home care groups, doctors, the municipality, and other care contacts. This provides good conditions for creating cohesive care and support, where different competencies are utilized.
