JOB DUTIES Engaged and confident nurse for home care in Bomhus and Brynäs. Do you want to work close to patients, make your own decisions, and make a real difference every day? We are looking for an engaged and confident nurse for home care in Bomhus and Brynäs. With us, you meet patients in their home environment and become an important part of their security and daily life. The work is varied, developmental, and independent while you always have the support of colleagues in the team. As a nurse in home care, you are responsible for planning, implementing, and following up on health and medical care measures in the patient's home. You lead the nursing work and are a central part of the coordination around the patient. Your tasks include: • Medical assessments and nursing responsibility. • Medication management and delegation. • Documentation and follow-up. • Collaboration with healthcare providers within the region and municipality. • Guidance and support for care personnel. You plan your workday independently and prioritize based on patients' needs. The role offers both responsibility and freedom. The position is based mostly on daytime work, but the schedule includes evening and weekend shifts. You will then work in our consulting operations, which includes the municipality's care and residential facilities, home care, and service and group homes within LSS and social psychiatry. Interviews may take place on an ongoing basis during the application period. QUALIFICATIONS You are a confident and clear nurse who builds trust when meeting with patients, relatives, and colleagues. Through your calm demeanor and professionalism, you lead nursing work with structure, handle changes with stability, and contribute to an open and developmental collaboration in the team. To be a candidate: • Licensed nurse. • At least 2 years of experience as a nurse, preferably within municipal health and medical care. • Driver's license and driving experience. • Experience with documentation. It is a merit for the position if you have experience with the systems: Treserva, TES, Lifecare, NPÖ, and Pascal.
