About the position
As a registered nurse, you are responsible for both scheduled and acute home visits to patients in the Inner City. You conduct medical assessments, plan and coordinate interventions, and collaborate with primary care centers, other care providers, and our call center.
The work gives you great opportunity to use your clinical experience and make independent decisions, while always having the support of an experienced team.
You work on a rotating schedule with day, evening, and weekend shifts (2 weekends out of 5). On weekdays during daytime, you work from our office at Sabbatsbergs Hospital where you assess incoming referrals, coordinate care interventions, and ensure patients receive the right care at the right time.
There may be an opportunity for office-based work Monday-Friday after the year-end.

Aleris Basic Home Health Care provides care on behalf of primary care centers in Stockholm County and offers medical interventions in patients' homes when the primary care centers are unable to make home visits.
We are part of Aleris Nearby Care, which also includes ASIH, specialized palliative care, Rehab Station, and Uppsala Nearby Acute Care.

We are looking for you who
Are a registered nurse
Have at least two years of professional experience
Have a driver's license (category B)
Have experience with TakeCare and good computer skills
Speak and write Swedish well
It is advantageous if you have worked in home health care, primary care, or acute care.
