About Us Aleris Basic Home Care operates on behalf of health centers and general practice clinics when they are closed – evenings, nights, and weekends. We provide patients with safe and secure care in their homes and operate from five locations across Stockholm County. We are part of Aleris Close Care together with ASIH, specialized palliative care, Rehab Station, and Uppsala Närakut. We are currently seeking a nurse for our Farsta office. Here you work in a defined geographical area and travel between patients in our environmentally classified service vehicles. The office is centrally located in Farsta with good public transportation. About the Role As a nurse with us, you'll have varied and developmental work where no two evenings are the same. You'll lead nursing assistants, conduct planned and emergency home visits, and are responsible for medical assessments and interventions in patients' homes. Part of your time is spent on referral management. Your work spans from simple care tasks to care involving advanced medical equipment and technology. Together with our call center, we also handle alarms from 1177 and SOS Alarm. Documentation is done in the TakeCare medical records system, and shifts are scheduled in Epsilon. You also have an experienced administrative team to help with planning and coordination, so you can focus on patients. Working hours are scheduled for evenings 15:00–23:00 and daytime weekends 07:00–16:00. You work 2 out of 5 weekends. You have: Category B driver's license Good computer skills Experience with the TakeCare medical records system Good Swedish language skills, both spoken and written It's a merit if you have experience in home care. As a person, you are confident in your professional role, professional in your approach, and have the patient's best interests in focus.
