We would appreciate it if you have experience or interest in, for example, geriatrics, frailty, coordination, and collaboration. You will work closely with our elderly care clinic, a multidisciplinary team with a geriatrician, physical therapist, social worker, and nurse, focusing on patients with complex and extensive care needs. Job Responsibilities The position combines patient care work with care coordination. You create structure and continuity for patients who need support from multiple professions or care providers. As a care coordinator, you work with tasks such as: Coordinate and follow up on care interventions based on patient needs Create continuity, security, and patient safety throughout the care chain Lead care plans (SIP) and collaborate with internal and external stakeholders according to ViSam Work with patients with frailty and complex care needs Help develop and shape the care coordination mission at Karla You will have an independent and varied role where you can use your expertise, see the bigger picture, and make a difference for patients. Qualifications We are looking for someone who: Is a licensed nurse or district nurse Is confident in independent medical assessments Is organized, takes responsibility, and enjoys collaboration Has a patient-centered approach and sees the whole picture Is curious, solution-oriented, and enjoys developing working methods Experience in primary care, geriatrics, care coordination, or collaboration is a merit - but we place great emphasis on your personal qualities, competencies, and interests. About the Operation Karla vårdcentral is centrally located in the western part of Örebro and has approximately 10,500 registered patients.
