JOB DUTIES
As a Silvia Nurse Specialist/Dementia Nurse, you work in a consultative, supportive, and developmental capacity across the entire service area. You are an important knowledge bearer and collaboration partner for both patients and relatives as well as for staff, primary care centers, and other stakeholders.
You will, among other things:
• Coordinate and monitor the flow around people with cognitive illness
• Participate in collaboration with primary care centers and other professions
• Perform memory tests and conduct home visits
• Provide support to patients and relatives through counseling, conversations, and educational initiatives
• Work actively with the BPSD registry and train both administrators and other staff
• Mentor and educate licensed personnel
• Participate in networks and collaboration forums at local, regional, and national levels
• Contribute to development and quality assurance within the municipality's care of cognitive illness
• Participate in improvement work, process management, and implementation of new working methods
• Keep educational materials current and disseminate new knowledge in the field
• Assist in work with action plans and long-term development of the municipality's care in cognitive illness
QUALIFICATIONS
We're looking for you who are a licensed nurse with further education in dementia/cognitive illness or other relevant specialist training. You have a strong interest in person-centered care and want to help develop dementia care in the municipality.
The position requires:
• Swedish nursing license
• Trained as a Silvia Nurse Specialist or specialist educated in care of cognitive illness/dementia
• Good documentation skills and good computer skills
• Good Swedish spoken and written
• Class B driver's license
It is advantageous if you:
• Are a certified trainer in the BPSD registry
• Have experience from municipal health and medical care
• Have experience in mentoring, education, and improvement work
• Are experienced in collaboration between different operations and professions
We're looking for someone who is confident in your profession and thrives in an independent role with great responsibility. You have good interpersonal skills, are pedagogical, and have an easy time creating trusting relationships.
You are organized, solution-focused, and have the ability to drive processes from idea to implementation and follow-up. The role requires that you can coordinate, inspire, and motivate others while working closely with the service.
