Rehabilitation coordination supports social workers and those working in financial assistance in contact with health and medical care. You work with people who are often far from the job market and have a need for in-depth care planning and coordination. The assignment is forward-looking and focuses on the individual's path toward self-sufficiency through, for example, rehabilitation measures, studies, or other appropriate interventions. We are now strengthening this work, in part related to activity requirements, and have close collaboration with both internal and external actors. The operations are organized into four units and you will be part of unit 3. We work together and have close collaboration between colleagues. You will also have close collaboration with the Cooperari project and the opportunity to contribute to the development of new working methods and solutions. 1 position(s). WORK TASKS As a rehabilitation coordinator, you coordinate, structure, and follow up on care and rehabilitation measures. You support people in contact with health and medical care, accompany them to medical appointments when needed, and conduct home visits together with relevant functions. You also initiate and coordinate Coordinated Individual Plans (SIP). You are in contact with AMA and Electronics Recycling to start work training and are responsible throughout the entire work training period, as well as registering for the Sikta measure. When receiving Sikta services, you as a rehabilitation coordinator remain involved as long as the need exists. You provide support and feedback to social workers and collaborate closely with, among others, our Labor Market Units, the Care and Services Administration, the Region, and the Public Employment Service. You also provide support in processes related to disability benefits and activity allowances. The role also includes contributing to operational development, knowledge dissemination, and preventive measures. You have an individually adapted and coordinating working approach with a focus on clear responsibility distribution and sustainable solutions. You have the freedom to manage your schedule based on the needs of the assignment and the individual's situation. QUALIFICATIONS We are looking for someone with higher education as a social worker, behavioral scientist, or other education that the employer deems relevant for the assignment. You have experience with collaboration with external actors, particularly the Region, the Social Insurance Agency, and the Public Employment Service. Current experience in public administration is highly meritorious. Experience with Viva and knowledge of and experience applying social law and administrative law are meritious, as is experience with Cosmic Link. You are familiar with independently gathering and assessing relevant information as well as compiling and documenting in individual cases from previous experience. As a person, you are flexible, independent, and curious. You are explorative in your working approach and dare to take initiative for new contacts, collaboration forms, and working methods. You enjoy collaboration and have a knack for creating structure and coordination. The position is limited to one year and starts in the fall of 2026, with the goal of starting no later than October.
