Job Description As a nurse, you will perform standard healthcare center tasks. In your role as rehabilitation coordinator, you are a resource and knowledge bearer for the patient, next of kin, healthcare providers on sick leave, and the entire team at the healthcare center regarding rehabilitation and sick leave matters. Licensed Nurse or District Nurse We are looking for a licensed nurse with experience working in primary care. Your tasks include prioritizing patient needs and providing care services. In acute situations, you will conduct systematic assessments according to the ABCDE model to quickly identify and address life-threatening conditions in an injured or acutely ill person. You will work in Tele Q or chat providing medical advice (in consultation with physicians) and nursing guidance via phone and digital channels to patients, colleagues in home care, other care providers, and ambulance services. You will work part-time at the clinic. Boxholm Healthcare Center has several different clinics. You will have several experienced colleagues who enable a secure knowledge base and daily learning. Rehabilitation Coordinator Rehabilitation coordinator is an important role in healthcare to support patients on sick leave in their rehabilitation process. The goal is to facilitate return to work or entry into the workforce. The rehabilitation coordinator's mission is to work in a coordinating capacity for patients on sick leave where care needs to be coordinated with other parties such as employers, the Swedish Social Insurance Agency (Försäkringskassan), the Public Employment Service (Arbetsförmedlingen), or municipal social services. Your tasks include providing support and advice to individual patients and conducting assessments of individuals' rehabilitation needs. You are expected to support and motivate the patient toward participation and personal responsibility in the sick leave and rehabilitation process. You are expected to be responsible for and participate in SIP/care plans and meetings with employers. Your tasks include supporting and facilitating the team and enabling clear rehabilitation plans. You can make professional assessments, initiate and conclude treatments, and have a good ability to organize your work.
