Job Duties
The work involves supporting patients on sick leave through the rehabilitation process and facilitating their return to work or entry into the workforce. The Rehabilitation Coordinator's support and coordination also serves as a resource for the physician and other healthcare professionals in rehabilitation and sick leave matters, providing support to the patient. The role also includes collaboration with external actors, such as the Swedish Social Insurance Agency (Försäkringskassan) and employers.
Employment Terms:
Permanent position. Full-time, 100% but part-time can be discussed. Working hours Monday-Friday.
A probationary period may apply. Start date by agreement.
Qualifications:
The Rehabilitation Coordinator must be licensed healthcare personnel or a social worker.
Experience working in Swedish healthcare, primary care is a merit. Good computer skills and experience with the TakeCare electronic health record system are merits.

- in addition to Swedish professional credentials, proficiency in Swedish at level C1 according to the Common European Framework of Reference for Languages (CEFR) is required. The same requirement is guiding for non-licensed personnel with patient contact.
