Psychosis Care Stockholm is a newly established healthcare operation within Stockholm County Healthcare Region. Here we consolidate care for patients over 18 years old with psychotic illness in a single organizational unit. We offer assessment, investigation, and treatment according to national guidelines and regional care programs. Within Psychosis Care Stockholm, we conduct development work, education, and research on psychotic illness. About Psychosis Clinic Södermalm We are a well-structured outpatient clinic with a passionate interest and commitment to patients with psychotic illnesses. We have excellent conditions to provide world-class psychosis care. For us, important values are patient focus, accessibility, good care, and patient-perceived quality. The clinic consists of specialist physicians, Case managers, a mobile team, and a nurse clinic. Additionally, psychologists, occupational therapist, physiotherapist, social worker, and secretaries work at the clinic. In total, we are just over 30 people. Your Tasks The working method is based on "Lower-intensity Case management in the form of a resource model" and emphasizes continuity and flexibility in supporting a patient group with varying care needs. A Case manager is responsible for planning, coordinating, and evaluating treatment and support for the patient. As a Case manager, you will also work with treatment individually and/or in groups based on the treatment offerings for the psychotic process. The role requires an independent, active approach to work in the different phases of the care process. Care is planned and carried out in consultation with the responsible physician and the multiprofessional team at the clinic. Cooperation with neighboring care units and social services occurs in most cases. Treatment is based on evidence-based methods and in accordance with national care programs and the process developed by SLSO for this target group. Qualifications You have relevant education in the field and are either a psychiatric nurse or assistant nurse with a psychiatry focus and/or have appropriate Case manager training. We are looking for someone with experience and routine working in psychiatric outpatient care, preferably in psychosis care. Experience/competence in treatment methods such as IPT-K, PNU, IMR, CBT, NECT is meriting. Personal Qualities You have the ability to work in a structured manner.
