By introducing integrated approaches, healthcare services and social services can be provided in a more person-centered and cohesive manner. This ensures that people with complex needs receive continuity, comprehensiveness, and participation in their care and support.
The work is based on the Flexible Assertive Community Treatment (FACT) model, and we are currently in an exploratory phase. To advance the development work, we are now seeking a process leader for FACT.

DUTIES
In the role of process leader, you will lead and coordinate the work of initiating implementation of the FACT model, in close collaboration with both the municipality and the region. You will be employed by Region Skåne, Adult Psychiatry, but have a collaboration assignment that means you also represent the six municipalities in northeastern Skåne.
Your duties include taking a driving and supportive role in exploring and, upon decision, beginning implementation of integrated approaches through FACT. The assignment involves creating conditions for joint decisions in a balanced and transparent manner and leading the process forward. You also plan for and/or participate in training and provide information about FACT in the sub-region. The target group for FACT work includes both children and adults.
You will work closely with process leaders with corresponding roles for development work in southeastern Skåne and with regional coordinators for comorbidity and mental health and suicide prevention. There is also a network of FACT process leaders in Region Skåne where knowledge exchange and methodology development are offered. The position is an initial project employment of approximately 1 year, with the possibility of extension. The main place of work is Kristianstad, but the position covers work throughout northeastern Skåne.

QUALIFICATIONS
We are looking for someone with an approved healthcare education or other university education we deem relevant for the position. If you belong to a licensed profession, Swedish language skills corresponding to at least level C1 according to the Council of Europe's level scale are required. For non-licensed professions, good knowledge of Swedish in speech and writing is required. Experience with collaboration and project management between the region and municipality is meriting. Furthermore, experience with FACT, work within psychiatry, and a B driver's license are meriting.
As a person, you are well-organized and used to working independently. You are also flexible and enjoy both collaborating and cooperating with others. You have good communication skills, are responsive, and can meet people in different roles and contexts. Furthermore, you work from a holistic perspective in your various processes. We place great emphasis on personal suitability.
