Within elderly care, we are in an exciting transformation process and are now seeking four care coordinators who want to be part of shaping the future of care together with us. Two positions are located at Kaitumgården, the municipality's largest care home, centrally located in Jokkmokk Municipality. The other two positions are located at Bjärkagården in Vuollerim. 4 position(s). RESPONSIBILITIES As a Care Coordinator in Jokkmokk, you will have a central role in supporting and guiding your colleagues in daily work, while ensuring that we meet our goals for quality and care. These positions do not include personnel responsibility, work environment, or financial management. The work consists mainly of standard care and nursing tasks. To be able to contribute with social support and guidance. To be a hub and be able to manage multiple threads at once. The Care Coordinator's primary responsibility is to ensure that daily care is provided according to established guidelines and that colleagues in the department receive support and guidance in their work. The Care Coordinator also has responsibility for developing and following up on work to ensure quality care for our residents. An important part of the role also involves being confident in teaching colleagues in a pedagogical way. You must be able to work independently and in collaboration with others in the work team. We use various digital tools, so you need to have knowledge/experience in using digital technology and be familiar with using a computer in daily work in the form of documentation. Since this role is new, there will be a clear job description for the position. Your immediate supervisor is the unit manager at the care home. QUALIFICATIONS Nursing assistant certification or other education/experience that the employer deems equivalent. We are looking for someone who is positive, loyal, engaged, solution-oriented, and responsible with good empathetic abilities. Strong knowledge of the Swedish language is required, both spoken and written, as communication and documentation are important parts of the work. Great importance will be placed on personal suitability, maturity, and teamwork abilities. Employment type: Permanent employment. Duration: Permanent position, start date: According to agreement.
