Care Coordinator for the Dementia Team Would you like to make a real difference for people with cognitive impairment? We are now seeking a Care Coordinator for the Dementia Team in Kungälvs kommun. We are a dedicated team with high expertise in cognitive care. About the Role As a Care Coordinator, you hold a central role in the Dementia Team. You support, mentor, and develop care staff to create safety and quality in care for people with cognitive impairment and BPSD (behavioral and psychological symptoms of dementia). You work closely with both clients and staff, and you are an important part of developing person-centered approaches where the individual's needs, resources, and wishes are at the center. Responsibilities You will: - Mentor and coach staff in daily work. - Introduce new staff members. - Provide support in complex situations and matters. - Work actively with person-centered care for people with cognitive impairment and BPSD. - Coordinate efforts between different professions. - Ensure that care plans are developed, followed up on, and used as a living working tool. - Lead training sessions, reflection meetings, and competence-building initiatives. - Monitor documentation, incidents, and quality work. - Contribute to the development of methods and approaches within cognitive care. What We're Looking For - You are a qualified specialist nurse (undersköterska). - You have experience working in elder care. - You have good knowledge of cognitive impairment and BPSD. - You are confident in your professional role and enjoy mentoring and developing others. - You have good collaboration skills and a solution-focused approach. - You are computer-savvy and have experience with digital social documentation. - You hold a B-level driver's license. - You communicate well in Swedish, both spoken and written.
