We are now seeking a dedicated care coordinator who wants to strengthen our team and create the best possible care for our residents. Our short-term residential facility for rehabilitation, assessment, and/or respite care as well as palliative care has 16 places. All have separate modern living spaces, large bright apartments in our newly built house. We also have large, nice communal areas and a large shared balcony. There is free parking outside the house. In the same building we have our nurses as well as occupational therapists and physiotherapists who instruct and delegate tasks for our residents in dialogue with the department staff. 🩺 About the Role As a care coordinator, you have an important function in the operation with a focus on quality, guidance, and individualized care. You work both strategically and close to the operation, with great responsibility for developing work methods and supporting colleagues in their daily work. Together with the Unit Manager, the role of care coordinator and its tasks for short-term services during the pilot project period will be evaluated and, if necessary, redistributed. 📌 Your Responsibilities Manage occupancy and care intensity statistics Participate in the admission process (registration and welcome meetings) Provide support in needs assessments for complex care needs Offer support and information to relatives Prepare, follow up, and quality assure care plans Ensure that routine and safety folders are updated Participate in daily care work with residents Guide staff, students, and serve as a method support Quality assure interactions and nursing competence Support documentation according to IBIC Prepare action plans for BPSD Collaborate with VET-trained resources and dementia teams Assist in incident management together with the unit manager Work with mentoring and guidance 🎯 Project Focus You work to create safe, dignified, and individualized services that promote independence and well-being for residents – while helping relatives feel confident that their loved ones receive good care. ✅ Qualifications Trained nursing assistant (undersköterska) Several years of experience in elderly care Completed mentor and guide training Good documentation skills according to current regulations Very good knowledge of Swedish, spoken and written Good knowledge of low-affective interactions Experience with IBIC/Life Care is a merit Experience from short-term services is a merit 💡 Personal Qualities Strong collaborative ability Pedagogical, calm, and solution-focused Ability to mentor, support, and guide Responsible and quality-conscious ⏰ Working Hours Monday–Friday, daytime Interviews may be held continuously during the application period. ✨ Do you want to make a real difference in people's lives?
