As an LSS case manager, you will become part of the assistance unit, which belongs to the social services administration. The assistance unit has 11 case managers as well as family support coordinators and senior guides. You will be part of a group of four case managers who work with cases in social psychiatry and LSS management, with your main focus being LSS case management. As an LSS case manager, you will investigate all LSS services, across all age groups. The unit is led by a unit manager and group leader with many years of experience in case management within the unit's area.

WORK DUTIES
Your task will be to investigate, assess, and make decisions on services according to LSS. You will work closely with the other case managers in the unit, with service providers, family members, healthcare, and other stakeholders. You will independently monitor developments in your professional area and participate as needed in the administration's development work.

We also invest in competence development to create conditions for you and your colleagues to succeed in achieving our goals. Several times a year, LSS case managers have received external competence development training in LSS case management. QUALIFICATIONS
We are looking for someone who is a social worker or has equivalent education, preferably with experience in LSS and SoL case management. It is meritorious to have experience working with people with disabilities, training and experience in IBIC, the Life Care management system, and knowledge in MI.
We expect you to be a responsible employee who can be flexible and find solutions with the individual at the center. We see that you need to be confident and have a good ability to handle different situations and meet people in different stages of life. You also need to have a good ability to prioritize. We also expect you to work independently, with a solution-focused approach, and be confident in making decisions.
Employment Type: Fixed-term position.
Duration: 6 months or longer, start date: As soon as possible or by agreement. One year substitute position.
