LSS Case Manager

The Disability Services Unit is part of the Authorities Division. The Authorities Division has approximately 70 employees who handle cases under the Social Services Act, the Act on Support and Services for Persons with Certain Disabilities (LSS), and the Housing Adaptation Grant Act. The unit also includes the Prevention Unit with family support and Health 65, and the division is responsible for fee administration. Together, we investigate, make decisions, and follow up on services for individuals who need support and care due to age or functional impairment – an important mission that makes a difference in people's daily lives. The Disability Services Unit has 19 dedicated and knowledgeable employees who handle cases under LSS, the Social Services Act, and housing adaptation grants. The operations are organized into four teams – two LSS teams focused on children and young people, and adults respectively, one team for housing adaptation grants, and social assistants. Your responsibilities As an LSS Case Manager, you work primarily under LSS, but also under the Social Services Act. Your work involves conducting investigation meetings with clients and/or their representatives, both in person and digitally. You investigate needs and make decisions based on laws, guidelines, procedures, and practice, and you follow up on decisions made. The investigations are conducted using the Individual's Needs in Focus (IBIC) method. Succeeding in this role requires a legally sound, attentive, and professional approach with the individual's needs in focus. An important part of the work is collaborating with other authorities and partners. We are looking for you who work based on Region Gotland's values – trust, care, and participation. Together with your colleagues, you contribute to a good work environment and actively participate in the unit's development and change work, including work related to comorbidity investigations and the new Social Services Act. Who are you? We are looking for someone with a degree in social work or other relevant academic education in social law. A class B driver's license is required for the position. We see that you have experience in case management and authority administration. You have experience with the target group. It is a merit if you have worked with the IBIC method and have experience with the Treserva business system. In this role, you need to be organized, thorough, and have very good communication skills both verbally and in writing. You feel comfortable in meetings, whether they are in person, digital, or by phone, and you build trust through your professionalism and personal maturity.
