Dietitian Unit Primary Care Paramedicin Sörmland About Us In Sörmland, all dietitians are organized within a dietitian organization that is part of the regional clinic Paramedicin Sörmland. Currently, there are nearly 50 dietitian colleagues in Sörmland. The primary care and specialist care units work closely together, creating opportunities to change your role during your employment if desired. The primary care dietitians' mission is to care for patients of various ages referred to us by the county's 31 health centers. Patient visits take place at the health center, and you will work closely with other professionals there, which promotes collaboration around patient care. Depending on the size of the health center, you may have a full-time position at a large center or a shared position covering two smaller centers. In Sörmland, we have a well-developed network of dietitian colleagues who meet regularly, enjoy a great sense of community, and are never more than a Teams call or phone call away. We are now seeking a new colleague for our primary care team with responsibility in western Sörmland, including Katrineholm, Flen, Malmköping, and Vingåker. The position we are filling will care for patients referred from the health centers in Flen and Malmköping. Your Responsibilities Our work is based on evidence-based guidelines and ethical codes. We work according to the nutrition care process, which includes assessment, investigation, diagnosis, and treatment of nutrition-related problems and documentation according to NCP. The work also includes assessing the need for and prescribing nutritional supplements. The work is varied and you will meet patients of different ages. Patient contacts can be individual at the clinic, via remote visits, or as group education. Dietitians may work as part of a team with other professionals at the health center for lifestyle-related conditions. Patient education for health center staff and collaboration with child health services are part of the role, as well as cooperation with municipal home care staff regarding patients with nutrition problems. Your Qualifications We are looking for a licensed dietitian, preferably with experience working in primary care. Strong Swedish language skills are required, both verbal and written. You should have an interest in working with the whole patient in collaboration with other professions, while bringing your nutrition expertise. You are organized and self-directed with good teamwork skills. Digital technology for communication is frequently used for video visits, so you need to have an interest in this and be willing to learn if you are unfamiliar with it. As travel may be required in the role, a Class B driver's license is desirable. We place great emphasis on personal suitability and flexibility.
