As a nursing care manager at Landskrona Omsorg AB, you are responsible for ensuring patient safety and high quality care for patients in home care, LSS residences, and care and residential facilities. Your main responsibilities include: • Nursing care responsibility - Assess, plan, implement, and follow up care interventions for patients with complex needs within municipal primary care. • Collaboration and communication - Cooperate with internal teams and external actors (inpatient care, primary care, ambulance) to ensure a cohesive care process. • Support and mentoring - Provide guidance and support to care staff, family members, and other care providers to promote quality and safety in care. • Documentation and quality assurance - Ensure proper documentation and reporting in accordance with laws, guidelines, and patient safety requirements. • Technology and medical technology - Handle medical technical equipment and use digital systems for care and administration. QUALIFICATIONS We are looking for you who are a licensed nurse and want to work with people in an ethical and professional manner. You have education and/or experience in: • Nursing care and medical technology • Trained nurse (district nurse, specialist nurse with a focus on elderly or general nurse) • Documentation and work in digital patient record systems • B driver's license for manual transmission is a requirement for the position You are welcome to have experience in: • Work in municipal care, palliative care, inpatient care, and/or primary care. • Collaboration with other care providers in municipal and regional care • Interaction with different cultures and the cultural differences it entails We believe that you are a person who: • Has fluent Swedish language skills, both spoken and written • Is confident in your professional role and has good interpersonal skills • Works in a structured manner and takes responsibility • Has good cooperation skills and finds it easy to build good relationships • Is solution-oriented and has technical understanding • Has genuine commitment to quality and patient safety In this recruitment, we ask you to attach your CV and answer a number of selection questions.
