Curator and Discharge Coordinator, Psychiatric Ward 5, Child and Adolescent Psychiatry
We are now developing the curator role at Child and Adolescent Psychiatry's inpatient ward 5 and are seeking someone who wants a varied assignment where you will be very much a key connector in the team. You will be surrounded by a work group defined by high competence and great commitment. Together with us, you will work on a very important mission: individual-focused care for children and young people up to 18 years old at a specialist psychiatric inpatient ward. Ward 5 is a child and adolescent psychiatric inpatient ward with 6 beds serving the entire Örebro region. We admit children and young people with serious psychiatric conditions for acute hospitalizations. Our patients are cared for together with their guardian(s), and the goal of the inpatient stay is primarily to stabilize the acute poor condition so that they can then return home and continue their care in outpatient services. On ward 5, we work in a positive and cohesive multiprofessional team consisting of the curator, nurses, care assistants, physicians, teachers, and the unit manager. Work Tasks As a curator on the ward, you participate in daily planning for our patients and participate in/conduct assessment and planning conversations with them. You also have primary responsibility for the parental support offered to all guardians. This support is important as we clearly see that the family network needs to be secure and sustainable for the young person to stabilize in their well-being. The curator profession is the only profession in healthcare with social-legal/juridical competence, and an important function is to coordinate interventions with various societal actors, safeguard and ensure patients' needs and rights are met, provide social counseling, and implement social measures including contact with social services. You will also be the ward's discharge coordinator, which means overseeing and facilitating safe care transitions, managing this planning to ensure that we and our care partners have the information needed for the patient and guardian to receive good and secure care throughout the entire care pathway. This work is conducted using the LifeCare system according to the ViSam model. Qualifications Licensed healthcare curator or social work degree. Experience working with children and families and knowledge of psychiatry is desirable. Psychotherapeutic basic training is meritorious, as is experience with LifeCare and the ViSam model. As a new team member, you will be offered a mentor for support in your work. The assignment is of a varied nature, so you need to have good structure in your work, experience managing documentation in a correct and efficient way, and good ability to express yourself both verbally and in writing. You should be comfortable taking responsibility and able to navigate multiple parallel processes. You need good initiative, collaboration skills, good judgment, flexibility, and the ability to make independent decisions. We value that you are open, unpretentious, and have a positive attitude, with the patient at the center. Contributing to the business's development and quality work you see as a natural part of your work. Great importance will be placed on personal suitability. Application The application should include a CV and cover letter. Certificates, grades, diplomas, etc. should be submitted at a possible interview.
