Welcome to Us Healthcare and Medical Care Team 7 is part of the healthcare and medical care function in Jönköping Municipality, consisting of 13 multiprofessional healthcare and medical care teams and one home care team. We have specialist services in psychiatry, palliative care, home rehabilitation, dementia, and disability care. Together we are approximately 460 dedicated employees. Our service area includes Bankeryd with countryside, Trånghalla, Kortebo, parts of Bymarken, and Järstorp. In the service area there are four elderly care facilities: Ceciliagårdens äldreboende, Ekåsens äldreboende, Ramsaygårdens äldreboende, and Vilhelmsrogårdens äldreboende. The work group operates from the team office at Strandängen and consists of occupational therapists, physiotherapists, nurses/specialist nurses, assistant nurses, and rehabilitation assistants. Your New Job As a physiotherapist, you work with home rehabilitation for patients in ordinary housing and specialized residential care. You meet people of different ages and life situations with varied rehabilitation needs, where no two days are the same. You are responsible for assessing, planning, implementing, and following up rehabilitative interventions based on the patient's individual needs. When necessary, you guide and educate relatives and care and support personnel. In this role, you have significant personal responsibility and the opportunity to plan your interventions based on the patient's needs and what works best for you and the operation. At the same time, you naturally have close support from colleagues and your manager. You work closely with both primary and inpatient care to adapt healthcare needs. In daily work, you collaborate closely with nurses, assistant nurses, occupational therapists, and rehabilitation assistants. Your Qualifications We are looking for someone who is a licensed physiotherapist. If you have prior experience from healthcare and care operations, we consider this a merit. Good knowledge of Swedish, both spoken and written, is a requirement. You also need to feel comfortable working in digital systems. A valid driver's license and experience driving a car are required as travel is part of the position. The work requires physical mobility as it takes place in home environments where situations arise that cannot be anticipated and must be solved in a manner appropriate to the situation.
