The Rehabilitation Unit is part of the Social Services Department. At the Rehabilitation Unit, we work in close collaboration with other personnel in care and support within the municipality's residential facilities, short-term housing, home healthcare, and home care services, as well as for individuals receiving LSS support. The Rehabilitation Unit serves all of Härjedalen and operates from the locations Funäsdalen, Hede, and Sveg.

Job Responsibilities
In your role, you are responsible for providing quality care and identifying and actively preventing health risks. You work with assessment, interventions, and follow-up for care recipients, and are an important and active part of the health and care team that constantly works with the individual's needs at the center.
Your responsibilities include:
Functional and activity assessments
Prescribing, fitting, and following up on assistive devices.
Designing and monitoring individual training and rehabilitation programs.
Guiding and training staff.
Collaboration with other professionals, relatives, and external consultants.
Documentation in the Viva journal system.
The work requires independence as you structure and plan your workdays on your own. Working hours are daytime, weekdays.
Qualifications
Requirements:
You are a licensed physiotherapist or physical therapist.
You have an interest in working within LSS and rehabilitation.
You hold a B-category driver's license.
To work in care for the elderly and adults with disabilities, a clearance from the criminal records registry is required.
Meritorious:
Experience from work within LSS, rehabilitation, or municipal rehabilitation.
Experience with assistive device prescription.
Experience in staff training.
In addition to adequate education and experience, personal suitability is given great weight.
