Hammarbygruppen offers short-term housing for people under and over 65 years of age, as well as permanent housing for people with dementia under and over 65 years old. At the short-term housing facility, we provide care with support under the Social Services Act (SoL) to patient groups who need the opportunity to recover after illness or injury, as well as other services. At our permanent facilities, we offer housing for people with a dementia diagnosis. Together we create a safe and stable living environment and adequate care for all stages of life for people with dementia. Hammarbyslott consists of 48 apartments in a home-like environment with nature right on the doorstep. The castle has preserved its beautiful architecture with high ceilings combined with modern standards and maintains large windows and generous open spaces for socializing. Examples of conditions include (Alzheimer's, frontotemporal dementia, multi-infarct dementia, and alcohol-related dementia). Hammarbyslott also has access to a day center. Job Responsibilities Your responsibilities include planning various activities/rehabilitation based on residents' resources and needs and leading these activities—individually and in groups. It is important if you have previous experience assessing and mapping activity capacity from a cognitive perspective, enjoy leading various group activities such as exercise classes and cooking, and have the ability to inspire groups. Continuous development work is ongoing in the operation, and we hope you will become part of it with us. The operation aims to maintain and strengthen the target group's functions through an activating and function-sustaining approach. As a physical therapist, you have a special responsibility for assessing the need for activation capacity and cognitive abilities, formulating goals, and establishing plans for support and training based on the individual's circumstances. The work also includes collaboration with activity coaches, residents, staff, and families. Qualifications We are looking for you to be a licensed physical therapist. It is a merit if you have previously worked in a similar facility. If you have completed dementia care training, it is an advantage. In your work, we see that you: Are self-directed and take responsibility for your tasks. Are attentive and relationship-building in interactions with residents and their families. Have good empathetic ability, good demeanor, are calm and positive in nature, have good collaboration skills, are flexible and innovative, and see opportunities. Work in equivalent operations or other similar activities is a merit.
