Welcome to us Healthcare Team 4 is part of the healthcare function in Jönköping Municipality, consisting of 13 interdisciplinary healthcare teams and one home care team. We have specialist focuses in psychiatry, palliative care, home rehabilitation, dementia, and disability care. Together, we are approximately 460 dedicated employees. HS-team 4 operates in the areas of Liljeholmen, Rosenlund, Österängen, Tenhult with surrounding countryside, and Ödestugu. Within our area are Österängen elderly care facility, Rosenlund short-term units, and Ekhagen short-term units. Our team operates from the team office at Kasernen near Asecs shopping center and consists of nurses, specialist nurses, nursing assistants, occupational therapists, physiotherapists/physical therapists, and rehabilitation assistants. Your new job As a pharmacist with us, you'll have a responsible and varied role focused on quality and safety in medication management. You'll be an important part of the healthcare function and will be responsible for: training care staff in medication handling for delegation participating with the team during admission of new patients for medication transitions maintaining routines in medication storage conducting medication procedure reviews for nurses playing an active role in implementing and managing medication reminders The role involves independent work combined with close collaboration in interdisciplinary teams consisting of nursing assistants, nurses, occupational therapists, and physiotherapists. Collaboration also occurs with external partners such as doctors, healthcare centers, and hospitals, and when necessary with family members. Together, we create conditions for continuity, efficiency, quality, and high patient safety, ensuring that each individual receives coordinated care and support. Your qualifications You are a licensed pharmacist or licensed pharmacy technician. Additionally, you have at least two years of professional experience from work in a pharmacy, clinical pharmacy, and/or municipal healthcare. Experience from care and support is a merit, such as work as a nursing assistant. You have good knowledge of Swedish, both spoken and written, and are comfortable with computers. Since travel is part of the job, a B-class driver's license is required. To succeed in the role, you are comfortable working independently and are confident in planning, structuring, and prioritizing your work based on operational needs. You have good collaboration skills and can adapt your communication and approach to different target groups and professional roles. You are thorough, quality-conscious, and flexible, with a genuine interest in both patient safety and development work, which are central to healthcare.
