Vårdcentralen Tollarp branch Degeberga are health centers located in the countryside, next to forest and open fields, and we have approximately 8,300 registered patients. At both locations there is a strong sense of community and great confidence in the health center. Here we offer care for both somatic and psychological complaints and work on developing new ways of working with team-based care. We have five nursing homes connected to us as well as home care services in two areas.
The doctor team consists of general practitioners and training doctors. In addition to doctor consultations, we also have physiotherapy, a social worker, dietitian, psychologist and occupational therapist, nursing assistants as well as nurse-led specialist clinics within asthma/COPD, diabetes, elderly care, child health care (BVC) and health conversations. At our specimen collection, experienced nursing assistants work, and in daily operations, medical secretaries, receptionists and an administrator are important support.
We are constantly developing and the focus is on developing teamwork further. We strive to be able to offer patients multidisciplinary assessments from professional roles such as physiotherapist, psychologist, social worker and occupational therapist based on needs. We actively work to utilize everyone's expertise fully. Through this, we can together provide the best care and use our resources optimally when the patient gets to meet the right person directly.

WORK TASKS
We now welcome a nursing colleague with a focus on asthma/COPD who also has a genuine interest in primary care.
As a nurse, you work with, among other things, your own clinic within asthma/COPD, telephone advice, patient triage as well as digital e-services and chat. Based on your previous experience and interests, other tasks may also be included in the position. We offer you stimulating work with varied tasks and good fellowship. As a nurse, you work independently and in close cooperation with your colleagues.
We base ourselves on the patient's needs, where care is designed individually and characterized by professional and competent treatment with high medical quality. As an employee with us, you will receive:

Duties may be conducted at both Tollarp and Degeberga.
QUALIFICATIONS
We welcome you if you are a licensed nurse, or alternatively a district nurse, with authorization to practice within Swedish healthcare. If you have further education in asthma/COPD, this is highly meriting for the position. Swedish language skills corresponding to at least level C1 according to the Council of Europe's level scale is a requirement. A valid B driving license is required as travel occurs in the position.
It is meriting if you have experience from working in primary care and/or experience working in the PMO medical record system.
