Piteå Health Center has approximately 10,100 registered patients and is one of Region Norrbotten's largest health centers. We work in three multiprofessional teams where doctors, nurses, nurse assistants, and rehabilitation personnel together care for their registered patients. We are now seeking District Nurses who want to work broadly within primary care and be part of a team where patients are followed over time and where collaboration between professions is a natural part of daily work.
What We're Looking For
You are a licensed district nurse with good knowledge of Swedish, both spoken and written. It is advantageous if you have experience from primary care, telephone advice, or your own practice. Experience or training in, for example, diabetes, asthma/COPD, or elderly care is also valuable. We are looking for someone who can make independent assessments, prioritize between different tasks, and who enjoys collaborating with other professions around patient needs.
What You Will Work With
As a district nurse, you will work with:
Telephone advice and medical assessments
Clinic visits with your own scheduled patients
Health promotion and preventive measures
Follow-up of patients with chronic diseases
Coordination for patients with comprehensive care needs
Collaboration with municipalities, specialist care, and other stakeholders when needed
You will be part of one of our teams and participate in team rounds with other professions. There, patient cases are jointly planned, prioritized, and followed up. The goal is for patients to meet a team that knows their situation and works in a coordinated manner.

Job Information
The position is full-time until further notice, working hours 08.00 - 17.00. Start date by agreement. Selection and interviews take place on an ongoing basis and the position may be filled before the final application deadline.
