About the Operation Primary care is the foundation of healthcare in Värmland, and we are here to provide good and accessible care to everyone in Värmland throughout all stages of life. Skoghall Health Center, located in Hammarö Municipality, is a well-functioning health center with experienced and dedicated staff. We have approximately 16,700 registered patients and about 50 employees. At the health center, different professions work together in teams to provide our patients with excellent care. We have good and close contact between our different professional roles, which is an important part of our operations. The health center is led by two unit managers. Together with inpatient care and municipalities, we work toward seamless care for our shared patients. Several physicians at Skoghall Health Center conduct research. This means we are involved in developing healthcare and creating better conditions for our patients by improving care quality. At the health center, we offer our patients physician consultations, diabetes clinics, asthma/COPD clinics, hypertension clinics, heart failure clinics, district nurse consultations, nurse consultations, nursing assistant consultations, vaccination clinics, counseling services, CBT, psychologist services, rehabilitation coordinator, and laboratory services. Your Responsibilities We are now seeking a district nurse/registered nurse who wants to work together with experienced district physicians, training physicians, district nurses, registered nurses, nursing assistants, care administrators, CBT therapists, and a psychologist. We follow patients through all stages of life. You will, among other things, work with your own clinic, infusion clinic, and telephone advice services. You will be part of a team with other nurses, physicians, care administrators, and nursing assistants. Your work also includes being part of our on-call services. At the health center, physiotherapists, rehabilitation coordinators, and Wellness Services in Värmland also work, with whom we have close collaboration. We have good and close contact between our different professional roles, which is an important part of our operations. Your Knowledge and Competencies We are seeking someone who is trained as a district nurse/registered nurse and has experience working at a health center. You are accustomed to working independently, are confident in your professional role, and are interested in providing high medical quality. You are responsible and have a structured way of working. You collaborate easily and want to actively contribute to our development, while demonstrating good patient interaction and strong organizational skills. It is advantageous if you have knowledge and/or further training in diabetes, asthma/COPD, heart failure, and/or hypertension.
