Welcome to Handen Health Center and Dalarö branch clinic! We are now looking for an engaged colleague to join Handen Health Center. Handen Health Center is located adjacent to Handen Center, with restaurants, grocery stores, and good public transportation. We also have rehabilitation and child health services as part of our unit. In Dalarö, in the beautiful archipelago setting, we have a small branch clinic that serves the population living there. Handen Health Center is growing and currently has close to 21,200 listed patients and is expanding rapidly. We have integrated home healthcare with approximately 200 patients enrolled and a responsible home healthcare doctor. We have a psychosocial team with a psychologist, psychotherapist, counselors, and a rehabilitation coordinator. Two foot care nurses. Nurse-led diabetes, asthma/COPD, hypertension, osteoporosis, and elderly care clinics. We have a minor orthopedic clinic working as a team with a physiotherapist and a minor acute clinic with close collaboration with the nearby emergency clinic. We operate an SMO clinic (coordinated medical care for persons with disabilities). We are one of four clinics in Stockholm offering this. We work in a well-functioning team with doctors, rehabilitation coordinator, district nurses, clinic nurses, counselors, nursing assistants, foot care nurses, and medical secretaries to create good continuity for our patients. In our work, we use TeleQ, chat, and video consultations, and we use Take Care as our medical records system.
