District Physician for Home Healthcare and Senior Living Care, Northern Värmland

We are seeking a district physician for work in home healthcare and senior living facilities, with responsibility for patients registered at Likenäs Health Center who are 75+ years old. Your Work Tasks As a district physician with us, you will work with: Medical comprehensive assessments of patients within home healthcare and senior living facilities. You work systematically with annual check-ups, medication reviews and make decisions regarding continued care level and any treatment limitations. You are responsible as a full-time specialist for a maximum of 1100 patients, with the patient list adjusted based on how much of your work time is allocated to home healthcare and senior living care. Health promotion and preventive work, especially for elderly people and those with chronic conditions. Collaboration with nurses and rehabilitation staff within municipal and regional activities. Participation in development work for equitable, safe and person-centered care. The work involves significant responsibility but also good opportunities for influence, flexibility and a high degree of independent work.
