Our offer Primary Care Center Östergötland consists of 33 region-operated health centers, four emergency centers, Health Center Finspång, the child health care unit, 1177 and a general medicine training center. We conduct broad primary care activities where we offer care to everyone throughout their lives; newborns, children, adolescents, adults, elderly and patients in their final stages of life. Health Centers Kisa and Österbymo are two well-functioning rural health centers within Primary Care Center in Region Östergötland, with close collaboration and a shared ambition to provide good and accessible care with high quality, availability and continuity. Together we are responsible for approximately 12,000 registered patients and offer primary care with great breadth, flexible solutions and well-functioning cooperation with municipalities' home care services. Job Description As a dietitian with us, you will get varied and independent work with placement at both health centers in Kisa and Österbymo. You will become an important part of our multiprofessional teams and work closely with patients and colleagues to promote health, prevent disease and contribute to good living habits. The assignment includes nutritional assessments, individually designed nutritional treatments, prescription of nutritional products and patient information both individually and in groups. You also serve as important support for colleagues in nutrition matters. Part of the position is focused on health conversations, where motivational conversation and guidance on lifestyle changes based on the patient's individual needs form a central part of the work. The position offers great opportunities for cooperation and development, and we welcome new ideas and initiatives so that together we can continue to develop the assignment and create the best possible conditions for our patients. Work Group Our operations play an important role in the community and meet a wide variety of medical issues, from preventive measures and follow-up of chronic diseases to more acute care needs. Due to the distance to the nearest hospital, primary care takes on a prominent and significant function, which creates both special challenges and great opportunities to develop safe and accessible care, also in patients' homes when needed. About you We are looking for you who are a licensed dietitian and have experience in your professional role; it is meritorious if you have experience from work in primary care. As a person, you are curious, engaged and have a solution-focused working approach. The work requires that you have good cooperation skills and enjoy being part of a team, but also that you have the ability to work independently. It is also important that you have a good ability and willingness to develop the operation and can take into account the larger perspective and see the greater good of the entire operation.
