JOB RESPONSIBILITIES As a diabetes nurse at Söderhamn Din hälsocentral, you are responsible for the daily operations of the diabetes clinic. You work independently with patient consultations while being part of a multidisciplinary team in close collaboration with physicians, rehabilitation staff, and other professionals to provide patients with safe and quality care. You are interested in development and improvement work and confident in your professional role, and you have the ability to interact with people in a professional and empathetic manner. In the role of diabetes nurse, you should • be a licensed nurse or district nurse with specialization in diabetes • hold a valid category B driving license. For this position, it is meritorious if you have experience working in primary care, experience with elderly team work, Lifecare, and the Cosmic medical record system.
