Piteå Health Center has approximately 10,100 registered patients and is one of Region Norrbotten's largest health centers. We work in three interprofessional teams where doctors, nurses, nursing assistants, and rehabilitation staff together care for our registered patients.
We are now looking for a diabetes nurse who wants to combine independent clinic work with close collaboration in interprofessional teams. With us, you will follow patients over time, work to promote health, and contribute to safe, coordinated, and person-centered care for people with diabetes.
What we're looking for
We are looking for you who are a licensed nurse with professional experience and good knowledge of Swedish, both spoken and written, equivalent to at least C1 level. You also have good computer skills. It is advantageous if you have experience working in primary care, running your own clinic, or in diabetes care, as well as knowledge of the Cosmic patient record system.
We place great emphasis on personal qualities. You are self-motivated and take responsibility for planning, prioritizing, and driving your work forward independently. You work in a structured manner and have the ability to organize and follow up your work with high quality. As a person, you have pedagogical insight and can adapt your communication based on the patient's needs and circumstances. You also have strong collaboration skills and enjoy working together with colleagues and other professionals to create the best possible care for patients.
What you will work with
As a diabetes nurse with us, you will work with your own clinic for patients with diabetes. You are responsible for assessment, counseling, patient education, and individually tailored follow-up of treatment, self-care, and lifestyle habits. The work is conducted in close cooperation with doctors and other professionals, and you will also contribute to the development of the diabetes clinic's working methods.
The position includes broad primary care work with telephone advice, medical assessments, acute and scheduled patient contacts. You will be part of an interprofessional team and participate regularly in team rounds to plan, prioritize, and follow up patient care together.

Job information
We offer you a permanent position with the possibility to work full-time or part-time. Working hours are daytime. Start date by agreement. Selection and interviews are conducted on a rolling basis and the position may be filled before the final application deadline.
