VC Avestahälsan has been operating since 2012 and currently has nearly 8,000 registered patients. We employ approximately 35 staff members including district physicians, resident physicians, assistant physicians, clinical assistant physicians, physiotherapists, occupational therapist, midwives, child health nurses, district nurses with various specialty areas, advisory nurses, medical secretaries, therapists, occupational therapist, and nursing assistants, along with our own laboratory.

We are a privately owned primary care center with a care choice agreement with Region Dalarna, meaning individual patients receive exactly the same conditions, care guarantees, and fees as at a publicly operated primary care center. VC Avestahälsan is independent of large healthcare companies and operates solely in Avesta, though we serve many registered patients from surrounding municipalities in southern Dalarna and northern Värmland.

DIABETES NURSE
At VC Avestahälsan, we have 6 district nurses, 2 midwives, and 4 additional registered nurses. Several are specialized and further trained in child health, diabetes, COPD/Asthma, dementia, incontinence, cardiac/vascular health, mental health, and rehabilitation coordination. We also receive assistance from several retired staff members who help out occasionally, primarily with vaccinations. Telephone advisory services are a major and important task for all our nurses, regardless of their specialty area.
One of our diabetes nurses will be retiring in March 2027, so we are looking for a diabetes nurse to join our great team! We are seeking a full-time registered nurse where approximately half the time will be as a diabetes nurse. We work on a team basis and set individual treatment goals for our patients.
Job Duties
Your job duties will include:
- Telephone advisory services.
- Regular outpatient clinic for our type 2 diabetes patients.
- Follow-up of these patients and medication dosage adjustments.
- Medication adjustments both under delegation and in consultation with physicians.
- Conducting motivational conversations about lifestyle habits.
- Providing patient education for pre-diabetes patients.

Licensed registered nurse
Diabetes nurse competence
Meritorious
District nurse
Experience working in primary care and/or healthcare advisory services.
