Would you like to help shape the future of dementia care in Höör Municipality? We are now recruiting an engaged and knowledgeable nurse with specialist training in dementia to our newly established dementia team – a team with high expertise and a central role in the municipality's development work.
About the Dementia Team
The dementia team will serve as the municipality's specialist and support resource in the field of dementia, consisting of a dementia nurse and a nursing assistant with specialist competence in dementia. Additionally, an occupational therapist, physiotherapist, and care coordinator will work closely with the dementia team. The team's mission is to ensure that people with dementia receive early support, coordinated interventions, and safe, evidence-based care. We work consultatively, with guidance, and person-centered – always with the individual's needs in focus.
The team leads the municipality's dementia work in accordance with the National Board of Health and Welfare's guidelines and the national dementia strategy. We collaborate with home care services, special housing, care coordinators, licensed personnel, and MAS/MAR.

YOUR RESPONSIBILITIES
Your work tasks
As a nurse in the dementia team, you will have a varied and developing role where you will work with:
Individual and family support
• Conducting home visits and assessments
• Identifying early signs of cognitive decline
• Supporting and informing family members throughout the disease course
• Collaborating in SIP (Individual Plan)
Specialist assessments
• Assessing symptom presentation including BPSD (Behavioral and Psychological Symptoms of Dementia)
• Conducting care assessments within the municipality's health and medical care responsibility
• Proposing individually adapted interventions
Guidance and competence development
• Guiding and training staff in person-centered working methods
• Working with the BPSD registry
• Participating in quality and improvement work
Coordination and development
• Ensuring that dementia care follows national guidelines
• Developing routines and working methods
• Participating in interprofessional collaboration
Working approach

• Structured according to the BPSD model – mapping, analysis, planning, and follow-up
• In a coordinated and interprofessional manner – together with the municipality's various operations
• Preventively and health-promoting – with focus on fall risk, nutrition, oral health, welfare technology, social participation, and safety
QUALIFICATIONS
We are looking for someone who is:
• A licensed nurse with specialist competence in dementia care
• Has good collaboration and pedagogical skills
• Experienced in municipal care and comfortable working independently and consultatively
• An excellent communicator with the ability to build trust and collaborate across organizational boundaries
• Engaged in development work and experienced in working in a structured manner
Meritorious qualifications include experience with BPSD work, guidance, and educational initiatives.
