Dementia Nurse for the Support Assessment Unit

We are now seeking an engaged dementia nurse for the support assessment unit in Upplands-Bro Municipality. About the Position: As a dementia nurse at the support assessment unit, you will have a central role in developing and ensuring early, coordinated, and needs-adapted interventions for people with dementia. Through your placement in the support assessment unit, you become an important part of the municipality's first point of contact – where you can contribute specialist knowledge from an early stage. You serve as a link between healthcare and the municipality's services, and work closely with support case handlers to create security and quality in support for the individual. In this role, you will, among other things: Provide advice, support, and guidance to people with dementia and their relatives Participate in home visits in early stages following diagnosis Contribute knowledge about the municipality's services, support available in society, and primary care's role Collaborate with support case handlers and other professionals to plan interventions Serve as a resource to the organization through competence development within cognitive illness, implementation of new knowledge and working methods, and participation in developing the organization You are also a resource for the entire elderly care services and can contribute to various parts of the organization as needed. About the Workplace: The position belongs to the support assessment unit within elderly care services, which is a strategically important entry point for interventions and support for people over and under 65 years old. Here we work close to residents and in collaboration with other functions to meet individual needs. The unit has 12 case handlers and two staff members, and you will report to the unit manager, while also having many opportunities for collaboration outside your immediate work group. We are Looking for You if You: Are a licensed nurse with further education in dementia or geriatrics. Have experience working with people with dementia. Have good knowledge and experience of working from a pedagogical approach and contribute to knowledge dissemination broadly. Speak and write Swedish fluently. Have a B-level driver's license. It is advantageous if you have worked in municipal elderly care and with support-related matters. As a person, you have a good ability to work both independently and in collaboration with others. You have excellent communication skills and are comfortable in meetings with both individuals and relatives in different stages of illness.
