Support and Prevention Services is part of the Elderly Care Department and is responsible for elderly care prevention work and authority functions. The office is centrally located in Huddinge Center, close to commuter trains and bus station. We offer developmental work in an organization where teamwork is a central part. Job Description As our current dementia nurse is leaving, we are now seeking a Dementia Coordinator. The position replaces the previous role of dementia nurse and involves continued and developed work providing support to people with cognitive illnesses, their relatives, and operations within the municipality. As a Dementia Coordinator, you work closely with various actors both within and outside the municipality. Internally, you work closely with case managers in elderly care, the municipality's family support, and preventive work with the elderly. You educate and support employees, monitor the external environment, and bring new knowledge to the municipality. Externally, you work closely with health centers in the municipality and memory clinics. You will participate in a network with dementia nurses/dementia coordinators from other municipalities. You educate and meet relatives of people with cognitive illness and have a strong interest in giving lectures when requested. You will be instrumental in developing early interventions for people with cognitive illnesses. Your Profile We are looking for you who are a nurse, social worker, occupational therapist, physiotherapist, or have equivalent higher education that the employer deems equivalent. You have further education or special expertise in dementia, geriatrics, or cognitive illnesses. You have experience working with people with cognitive illness and a good understanding of the target group's needs and the challenges that relatives may face. You have documented experience working on projects or development work and have good computer skills and an interest in designing and conducting training. To succeed in the task, you should enjoy teaching knowledge in various forms. You see opportunities in welfare technology and digital working methods that can contribute to increased quality, participation, and independence for people with cognitive illness and their relatives.
