Develop With Us We're seeking a case manager for the social psychiatry target group within the adult secondary services area in Haninge Municipality. This is a temporary position running through August 2027 with potential for extension. Adult secondary services include authority functions for young adults, adults with substance abuse issues, and social psychiatry. We operate our own residential facilities for these target groups, residential support, community-based services, and open care services. Social Psychiatry Assessment works with authority functions under the Social Services Act. The unit's overall goal is to create conditions for increased independence for individuals in the target group. Operations are conducted in accordance with legislative requirements, political objectives, and municipal guidelines. Together, we work toward the administration's common goals. We are accessible and provide good service; we treat our residents with professionalism and respect; and we carefully manage taxpayers' money. The unit's focus in authority functions is legally sound case handling and consistent assessments. The unit's mission is to handle applications from individuals, assess needs for services, make decisions, and follow up on services. The unit consists of seven case managers, three administrative staff serving the entire area, and a unit manager. Your Assignment You will primarily handle cases and assess the need for support and services for people with mental health disabilities. The target group consists of people aged 18-64 who primarily have mental health disabilities. We generally work at the administration to develop new approaches to strengthen work with people with comorbidities. Examples of work tasks: Receive applications and conduct assessments to map individuals' service needs and make case decisions Collaborate both externally and internally, lead SIP, care planning, and other network meetings Document relevant information Stay updated on current legislation Contribute to the unit's general development and foster a good work environment What We're Looking For Sociology degree or currently in final semester of sociology studies Theoretical knowledge of current legislation Experience with authority functions and case handling in the area of mental health disabilities Strong ability to communicate clearly both verbally and in writing, with excellent Swedish language proficiency It's important to us that you are a stable person who can stay calm in difficult situations. You also have a solution-oriented and empathetic approach and are clear and structured in your work. You are self-motivated and able to take initiative to ensure work runs smoothly. You have strong collaboration skills and place great value on providing good service, both internally and externally.
