Together, we create safe and quality healthcare and social services for our residents. We are now seeking a Social Support Case Manager for the planning team at the Support Services Unit. As a Social Support Case Manager, you are a key person in our work to create safety, quality, and dignity in people's daily lives. 1 position(s). WORK TASKS As a Social Support Case Manager, you work close to people in different life situations and help design interventions that make a real difference for our residents. The work is both independent and team-based, and you get to use your expertise to find solutions that are both sustainable and legally secure. In the planning team, you have a key role in the transition between hospital and home. Your main task is to meet people who are cared for in the hospital and together with them plan appropriate support and services according to the Social Services Act before discharge. You will also follow up and meet people who stay at short-term placements, where you ensure they receive the support they need and that the interventions work in practice. Your work tasks include, among other things: • Receive applications according to SoL and investigate the individual's need for help. • Make decisions about services or rejections based on the investigation results. • Work with customers who are in the hospital and need services to be discharged. • Collaborate with customers, relatives, and other professional actors. • Follow up and evaluate interventions to ensure good quality. • Work independently with your own cases, but also close to colleagues in the team. As a Social Support Case Manager, work may be placed within the team where needs exist in the operations. QUALIFICATIONS Who are you? We are looking for someone who has: • A degree in Social Work, Behavioral Science, or other education that the employer considers equivalent. • Class B driver's license. • Previous experience in social support case management.
