Together we create hope for the future. About the Position The Case Management Unit in Vilhelmina Municipality is now recruiting a Case Manager for a full-time permanent position. A probationary period may be applied. We're looking for someone who is engaged, proactive, and solution-oriented. Your main responsibilities include: Investigating, assessing, and making decisions according to the Social Services Act (SoL) and the Act on Support and Services for Persons with Certain Disabilities (LSS) Following up and reassessing interventions Maintaining contact with individuals, relatives, and other stakeholders Collaborating with operational managers in elderly care and LSS services Cooperating with primary healthcare, the regional authority, and other government agencies Contributing to the development and quality work of the organization The target group includes both people under 65 years with disabilities and elderly people over 65 years. Qualifications Social Work degree or other education that the employer deems equivalent and relevant for the field Strong ability to communicate in speech and writing Class B driver's license It is meritious if you have: Experience in case management or similar official authority work Knowledge of current legislation Experience in elderly care or LSS services Experience with the Treserva management system As a person, you are confident, attentive, and responsible. You collaborate easily while also being able to work independently and make your own decisions. You want to contribute to a positive work environment and are motivated by developing the organization together with your colleagues. Great emphasis will be placed on personal suitability.
