The municipality is organized in one administration with four sectors. The Social Services Sector is undergoing an organizational change where we are expanding from four operational areas to five areas: Special Housing/Short-term Activities, Home Care, Individual and Family Care, Disability Services, and the fifth area Municipal Primary Care/Assistance/Prevention Unit. At the Assistance Unit/Care and Services, 8 social workers, senior social worker, and unit manager work together. The Assistance Unit consists of a knowledgeable work group where collaboration and teamwork are fundamental to our way of working. WORK DUTIES We are now recruiting two social workers to our team as two valued colleagues have moved on to new assignments within the municipality. As a social worker, you will handle applications, investigate, assess needs, provide information, make decisions, and follow up on interventions for those entitled to support under the Social Services Act (SoL). We work to meet people in need of support, including both seniors and younger people with different support needs in their daily lives. We collaborate with service providers such as home care, special housing facilities, and other social services, authorities, and healthcare. The work involves a high degree of official decision-making with requirements for legal certainty, consistent approach, and equitable assessments. With your knowledge and good service approach, you ensure that the individuals you meet receive the help and support they need to live a good and independent life like others. We use the documentation system Procapita/Life Care and the Assistance Unit has been working according to IBIC for a few years. The assignment primarily involves managing the care planning system SAMSA and the admission and discharge process. We work with planning teams where we collaborate with several relevant stakeholders to ensure safe discharge from hospital care. The division of work within the group is that three colleagues work in planning teams and the remaining staff handle other case management. We are seeking social workers who are curious and engaged about the changed work methods that are coming with the new Social Services Act. QUALIFICATIONS We are looking for someone who is a social worker (socionomexamen) or has other higher education that the employer considers equivalent. Meritorious: • Experience with official decision-making and preferably within elderly care • Familiarity with admission and discharge processes from inpatient healthcare • Knowledge of IT services SAMSA and Life Care • Personal suitability Since the positions involve administrative work, you need to have a good ability to express yourself in Swedish in both speech and writing. To suit the role, you need to be thorough, structured, and efficient with your time. You work well with others by listening, showing interest, and respect. You understand the shared mission and work to ensure it is carried out well, with both users, family members, and your work group in mind.
