Bra Liv Habo Primary Care Center is part of the primary care group Vårdcentralerna Bra Liv and provides primary care at 28 primary care centers within Region Jönköpings län. We collaborate within the group through knowledge sharing, shared resources, and work together to ensure you can always reach us. Bra Liv Habo provides primary care in close collaboration with our neighboring healthcare services in Habo Municipality: home healthcare, elderly care facilities, and family centers including preventive child and maternal health services. The role of Occupational Therapist and Rehabilitation Coordinator We at Habo Primary Care Center are now seeking an engaged occupational therapist and rehabilitation coordinator for a permanent position. We would appreciate if you have good knowledge of Bra Liv and experience in independent work as both an occupational therapist and rehabilitation coordinator in primary care, as well as experience working in Cosmic and rehabilitation support systems. You enjoy working independently but also appreciate and participate in the team's shared mission. You have expertise and interest in hand rehabilitation. You have excellent communication skills and see dialogue as an important tool in your contacts with patients, employers, the social insurance agency, and other colleagues at the primary care center. Bra Liv Habo Primary Care Center has approximately 39 employees and 10,700 registered patients. You will have both occupational therapist and rehabilitation coordinator colleagues on site. You are part of the primary care center's rehabilitation team and work closely with other professions at the center. The rehabilitation team consists of an occupational therapist, physiotherapist, psychologist, social worker, and rehabilitation coordinator. Development work is something we prioritize. You also have access to shared professional development activities with Vårdcentralerna Bra Liv's other occupational therapists and rehabilitation coordinators. Your profile You are a licensed occupational therapist with experience working as a rehabilitation coordinator, preferably in primary care. As a person you should be able to work independently and with different professions. Job responsibilities include rehabilitation coordination, hand rehabilitation, and group activities such as ReDO and Ha Koll. The work is varied and fast-paced, so it is important that you are flexible and adapt your work, approach, and attitude based on colleagues, patients, and situations.
