Work Tasks: The work tasks include: Activity and functional assessment, investigation/training, advice and support regarding self-care Prescription of assistive devices and information about products and technology Cognitive assessments using Mindmore Collaborate with other healthcare providers, guide and support relatives and care staff, and supervise students Work is carried out primarily in the patient's home Documentation is done in the Take Care journal system Type of Employment: Permanent employment.
