We provide primary care with physician services, respiratory clinic, home visits, child health services, district nurse clinic, psychosocial team, occupational therapy, and physical therapy. JOB RESPONSIBILITIES
Are you an occupational therapist who wants to work with us to support our patients' opportunities for activity and health? The position is offered at 100 percent. The need at Hedesunda Färnebo Din Health Center is 50 percent, and the remaining 50 percent involves working in the team within the primary care collaboration area Gävle.
As an occupational therapist with us, you will work with, among other things:
• assessment of functional capacity in consultation with the patient and need for rehabilitation measures
• assessments, prescription of assistive devices, fitting, and follow-up according to the prescription process
• memory evaluations as a complement to the district nurse's and physician's assessment/evaluation
• formulating statements in matters concerning housing adaptation and applications for personal assistance
• monitoring assistive device costs and rental responsibility
• patient education and counseling
• individual treatment to a high degree in collaboration with other professionals
• assessments and occupational therapy interventions in the patient's home.

QUALIFICATIONS
We are looking for a flexible and engaged colleague who enjoys being part of the development at the health center and within primary care. You are responsible and solution-focused with good collaboration skills, as we have interprofessional collaboration.
In the role as an occupational therapist, you should:
• be a licensed occupational therapist
• hold a valid B driver's license.
For this position, it is an asset if you have experience working in primary care and/or home health care.
